Insurance Information

As a courtesy to our patients, we will bill your private health insurance—we do not charge for filing insurance claims.

* Sloans Lake Managed Care

To efficiently and accurately process claims, we must have complete and correct information regarding your coverage in our records. Please contact the billing department if you need assistance or have questions concerning your insurance coverage.

We can be reached at 970-375-3619 or toll-free 1-800-524-9821 for billing questions.

Office hours are 8:00 am to 5:00 pm, Monday through Friday.

We request that you pay for all office visits, X-rays and supplies at the time services are rendered. We will be happy to bill your insurance company for you and request that all co-pays be paid at time of sevice and co-insurance within 30 days of the date on statements you may receive from us. We accept cash, personal checks, Visa, MasterCard, American Express and the Discover Card.
